New Patient Registration

As part of the process of registering with us we will ask you to attend a "New Patient check" with one of our healthcare assistants.  This helps us to get to know you and your medical history.

Our aim is to treat you with the respect you deserve and we expect you to treat us in a similar way.  Action will be taken against any patient who is violent or abusive to any of our staff or other patients whilst on our premises.  Ultimately, this could result in you being removed from our list. You may also be asked to leave if you move out of the area, continually not turn up for booked appointments, or if there is a breakdown of the doctor patient relationship.
